You may fill out these forms electronically and email them to employeeservices@hr.linde.com . Alternatively, you may print and email a scanned copy.

Superannuation:

BOC has a preferred superannuation fund arrangement with Equip, a profit to member industry superannuation fund. The Company pays for all administration fees and insurance premiums for the standard insurance benefits for BOC employees who join Equip. Please complete the 'BOC Superannuation Selection Form' and 'ATO Standard Choice Form' (or similar super fund nomination form) and email to employeeservices@hr.linde.com .
